Planet Beach Spa Franchise Case Study | Brand Journalists

Steven Smith, CEO and founder of Planet Beach, a popular spa franchise with 300 locations, sagged in his chair when he read the January, 2010 edition of Entrepreneur Magazine. The magazine, which publishes an annual list of the top franchise systems and reports on trends in the franchise industry, listed ?tanning franchises? in its list of what?s not hot for 2010.

The problem?

Planet Beach started out as a tanning franchise and quickly grew into a vibrant and prosperous franchise system. In 2004, Smith had the vision and foresight to change the business model and switch the system from a focus on tanning to automated day spa services ? push-button spa treatments performed in private rooms.

The new model, which he calls the Contempo Spa, started a movement in the day spa industry. Automated day spas, which customers pay monthly memberships to join much like a health club, became popular. Customers loved that they could visit the spa for one low monthly fee as often as they wanted. The model took off and the franchise grew while the tanning industry began to diminish.

Entrepreneur Magazine is considered the bible for most business owners and it carries and amazing amount of influence for franchise systems. In the world of franchising, public perception of a brand does more to determine what brands grow and which ones struggle. The January 2010 trends article proclaimed that tanning franchises were out and listed Planet Beach as a tanning franchise.

For Smith, this was problematic. Franchise leads for new locations dropped way off and customer perception of the brand focused more on the tanning heritage of the brand vs the innovative day spa franchise it had become.

The solution?

Planet Beach engaged Brand Journalists to redesign its franchise development website, launch an aggressive business blogging effort, launch social media for franchise development and begin national PR to create buzz and build brand awareness.

A Planet Beach Spa Service

The goal: change public perception of Planet Beach from a tanning franchise to a growing and popular day spa franchise and begin generating higher quality engaged leads for franchise development.

Results: In January 2011, one year later, Entrepreneur Magazine listed as What?s Hot in Franchising for 2011: Day Spa Franchises. The magazine listed Planet Beach as a best bet in this growing segment. INC magazine followed suit shortly thereafter. Ask people who work in the franchise industry, and they?ll tell you that Planet Beach is a forward thinking franchise system with good numbers that caters to a really attractive niche.

Throughout 2010, Planet Beach?s new franchise development website, www.myplanetbeachfranchise.com, gained larger and larger numbers of visitors. Pulled from aggressive blogging and organic SEO monitoring with social media mentions, leads began coming in again and sales began to take off.

In 2012, Brand Journalists added a downloadable brand story document that potential franchise candidates could download in return for filling out a lead form. The document, called a franchise report, was a success and lead flow doubled from 2011 to 2012.

Throughout 2011 and 2012, company website leads made up almost 70% of closed franchises, out performing all other advertising sources. Leads were better educated, more engaged and willing to respond to recruiter phone calls and emails. The number of closed franchise sales increased sharply from 2010 to 2011, despite the difficult economy and scarcity of business lending.

Report: Irish author Maeve Binchy dies at 72

LONDON (AP) ? Bestselling Irish author Maeve Binchy, one of Ireland's most popular writers who sold more than 40 million books worldwide, has died in Dublin after a brief illness, according to Irish media reports. She was 72 years old.

She was best known for her depictions of small-town Irish life.

The Irish Times, her former employer, told the AP it had spoken to Binchy's family and said the acclaimed author had died in a Dublin hospital on Monday with her husband Gordon Snell by her side.

Binchy, author of "Circle of Friends" and "Tara Road," wrote 16 novels, four collections of short stories, a play and a novella. Her work landed her on The New York Times' bestseller list and in Oprah's Book Club.

Describing her childhood in Ireland, Binchy wrote on her official website that she was "full of enthusiasms, mad fantasies, desperate urges to be famous and anxious to be a saint. "

After graduating from University College Dublin, Binchy worked as a teacher before becoming a journalist, columnist and editor at the Irish Times, one of the country's leading newspapers.

She later moved to England, where she became the newspaper's London editor in the early 1970s.

Her first novel, "Light and Penny Candle," was published in 1982 and went on to become a bestseller.

Several of her works ? including "Circle of Friends" and "Tara Road" ? were turned into films. "Tara Road," about Irish and American women who switch homes without having met, was chosen by U.S. TV talk show host Oprah Winfrey for her popular book club, bringing her many new readers.

Binchy's last novel ? "Minding Frankie" ? was published in 2010, the same year she received a lifetime achievement honor from the Irish Book Awards.

(AP) ? The two Republicans vying for the U.S. Senate nomination in Texas spent the final hours of their white-hot runoff race rallying their bases Monday, with Lt. Gov. David Dewhurst making a hard appeal to veterans and tea party-backed Ted Cruz taking his anti-establishment message to radio and television stations.

The race is being watched nationally as a test for the tea party against well-entrenched Texas Republicans who aren't used to losing. Texas hasn't elected a Democrat to statewide office since 1994. Whichever candidate wins Tuesday's runoff will be an overwhelming favorite against the Democratic nominee in November for the right to replace retiring Republican U.S. Sen. Kay Bailey Hutchison.

Dewhurst, who once looked like a slam-dunk for the job, is now believed to be in a major fight after failing to win a majority in the nine-candidate GOP primary on May 29. He has overseen the Texas Senate from the powerful lieutenant governor's post since 2003 and has been endorsed by popular Gov. Rick Perry and much of the state's Republican establishment.

Cruz is the son of a Cuban immigrant who has a fiery public speaking style that has fired up grassroots groups.

An Air Force veteran who also was stationed in Bolivia for the CIA, Dewhurst spoke Monday in San Antonio at one of the oldest Veterans of Foreign Wars posts in the country, flanked by Perry and Peter Holt, the owner of the city's NBA franchise, the Spurs.

His voice cracked as Dewhurst described his father, Lt. Col. David H. Dewhurst, who was a World War II pilot who was home from the fighting when he was killed by a drunk driver when his son was just 3.

"This is a crusade. My dad flew 85 missions. He didn't have a chance to fly his 86th," the lieutenant governor said, close to tears. "Let's make this his 86th mission."

His comments came after retired U.S. Navy fighter pilot Jake Ellzey said at the same rally, "I dare Ted Cruz to come in to any VFW post anywhere in the state and talk about fighting."

"Mr. Cruz, you're not a fighter because you've never worn the uniform," Ellzey said.

Asked if that was a fair assessment, Dewhurst said, "I agree with the commander."

"Normally, I've found in my life that the louder you speak, probably, the less of a fighter you are," he continued. "All the fighters I've had a chance and the honor to serve with didn't pat themselves on the back, were pretty humble men and women."

Cruz describes himself as the race's only true fighter for conservative values. A spokesman for his campaign, James Bernsen, said Monday that Ellzey's and Dewhurst's comments offended him as a veteran of the Iraq war.

"I find it offensive to make it a litmus test," Bernsen said. "Ted is very supportive of the military. But this is not something that should be politicized, period."

Cruz has received millions from national conservative groups including the anti-tax Club for Growth, which have targeted Dewhurst as too moderate for occasionally working with Democratic state senators to ensure key legislation got approved over the years. An energy business owner, the lieutenant governor has lent his campaign $24.5 million.

At Monday's rally, Ellzey addressed the out-of-state support for Cruz, saying that "1.8 million Texas veterans know that a slash and burn mentality as espoused by the tea party is only going to hurt our veterans."

Dewhurst and Perry then headed to Dallas for a fundraiser with top Republicans donors. They also appeared at a Dewhurst phone banking center in the city with the Republican primary's third-place finisher, ex-Dallas Mayor Tom Leppert and former NFL running back and ESPN commentator Craig James, who finished fourth. Both have since endorsed Dewhurst.

Cruz, meanwhile, made 11 appearances on radio stations throughout Texas and also had a string of television interviews, including addressing a national audience on Fox News Channel.

Early voting ended Friday, and about 3.3 percent of registered Republicans cast ballots ? a stronger turnout than expected, especially for a runoff coming so deep into summer doldrums. The initial primary was delayed from Super Tuesday to May because of a legal battle over redistricting maps drawn by the Republican-dominated Texas Legislature.

"We are seeing a high turnout which I think is terrific because everything we're seeing is that conservatives are energized and excited and showing up to vote," Cruz said on the Joe Pags Show on WOAI radio in San Antonio. "This race will come down 100 percent to turnout."

Voting totals in Harris County ? which includes Houston, where both Dewhurst and Cruz live ? were solid, with more than 70,000 GOP ballots cast early, while nearly 34,000 Republicans voted early in Dallas County.

That could be good news for Dewhurst, who won Dallas County with about 24,500 votes during the primary. But also receiving strong support there in May was Leppert, who took 23,000 primary votes. If Leppert voters now go for Dewhurst, higher turnout could be good news for the lieutenant governor in Dallas County.

Cruz, meanwhile, won Tarrant County, which includes Fort Worth, during the primary. Nearly 35,000 people voted early there in runoff balloting.

Port Angeles High School boys basketball coach resigns

PORT ANGELES ? The Port Angeles boys basketball team is without a head coach after Wes Armstrong resigned this week to take a teaching job at Kelso High School.

?It was with a heavy heart,? Armstrong said of his resignation during a telephone call Friday. ?I made this decision with my family in mind.

?In a perfect world, I would spend my entire career here.?

Armstrong, who guided the Roughriders to a 48-25 record in his three seasons at the helm, applied for a teaching job at Port Angeles High School.

When he wasn't hired, he had to look elsewhere for a place to teach special education. Kelso hired Armstrong soon after receiving his application.

Though it didn't end in the manner he would have preferred, Armstrong is content with his tenure.

He said when he was hired in 2009 he was asked to rebuild a struggling program and is satisfied with what he accomplished.

The Riders were 3-15 the year before Armstrong arrived. He led them to season records of 12-11, 19-7 and 17-7.

They made the state tournament in 2011 and were one win away from another trip to state last season.

When reached Friday, Port Angeles athletic director Dwayne Johnson expressed sadness to see Armstrong leave.

?He did an excellent job,? Johnson said. ?We're impressed with his performance, character and ability to coach.?

Armstrong said his proudest moment was beating Sequim in February 2011 on the night the 1966 Port Angeles boys basketball team was honored.

The win clinched the No. 2 seed at the Class 2A sub-district tournament.

?To do it not only in front of our fans but that great Roughriders team that took second at state, that was the pinnacle,? Armstrong said.

Though he leaves the Riders with no regrets about the work he did, Armstrong's early exit means some business is left unfinished.

?I always wanted to win a league championship or bring home some state hardware,? Armstrong said.

Armstrong expressed his gratitude to the players, parents, coaches and the staff at Port Angeles for their help and support.

He said he notified the Port Angeles players about his resignation through email and letters, and that he expects to have a short meeting with them Monday night.

At Kelso, Armstrong returns to the school where he served as a volunteer assistant coach in 2008-09.

But he plans to take a year off from coaching, and said a break might do him some good.

?It might be nice to be away from basketball for a year and recharge the batteries a bit,? Armstrong said.

Johnson said the search for Armstrong's successor will begin as soon as possible.

He added that he will first look within the district for a new coach.

Casey Hampton, Alameda Ta'ammuAP

When Pittsburgh opened last season by taking a one-sided beatdown in Baltimore, Warren Sapp proclaimed that the Steelers were ?old, slow and it?s over.? Casey Hampton is, at age 35, the second-oldest Steeler. And at well over 300 pounds and coming off a torn ACL, he may be the slowest Steeler as well. But he insists that it?s not over.

?They?ve been saying that for how long?? Hampton said, via the Associated Press. ?They?ve been saying, ?You?re getting old,? and all that for years, and year in and year out we do our thing. So I don?t see it being any different this year.?

The Steelers aren?t as old as they used to be, with aging veterans James Farrior, Aaron Smith, Chris Hoke and Hines Ward retiring this offseason. Hampton said the Steelers have the right players in place to replace those longtime teammates.

?No question about it, when you?ve been around those guys 10-plus years, it?s definitely different,? Hampton said. ?But that?s part of it. You see guys come and go all the time. Those are definitely special guys, but I think those guys will fill in and do their best to make up for it.?

Hampton is currently on the Physically Unable to Perform list, but he says he?ll be ready to go when he needs to go.

?I don?t have no problem with being on it as long as I need to be,? Hampton said. ?I just want to get it right and be right when I?m out there. I don?t want to be out there hobbling messing around; I wanna be out there ready to go.?

And Hampton thinks the Steelers are ready to go to the postseason again, regardless of anyone saying he?s old and slow.

New cancer drugs: Fitter, happier, more productive? Or not ...

It?s known as ?targeted? therapy, and it?s the holy grail of cancer research these days. If you listen to its most vocal proponents, it?s the path towards ?personalized medicine? that improves survival with much lower toxicity, in which, instead of using the hammer that is chemotherapy, precisely targets specific molecular abnormalities that drive cancer growth. With the advent of the revolution in genomics that has transformed cancer research over the last decade, including the petabytes of sequence and gene expression data that pour out of universities and research institutes, the promise of one day being able to a patient?s tumor, determining the specific derangements in its genome, gene expression, and protein expression that drive its uncontrolled proliferation, and finding drugs to target these abnormalities seems more tantalizingly close than ever. Indeed, it seems so close that even dubious practitioners, such as Stanislaw Burzynski, have jumped on the bandwagon, co-opting the terms used by real oncologists and real cancer researchers to sell ?personalized gene-targeted cancer therapy,? which in their hands are really no more than a parody of efforts to synthesize the enormous quantity of genomic data each patient?s tumor possesses and figure out how best to take advantage of it, a ?personalized genomic therapy for dummies,? if you will.

That?s not to say that there aren?t roadblocks to realizing this vision. The problems to be overcome are substantial, and I?ve discussed them multiple times before. For example, just a couple of weeks ago I discussed an example of just what it takes to apply these new genomic techniques to an individual patient. The resources required are staggering, and, more problematic, there often aren?t any single ?magic bullet? molecular pathways identified that can be targeted with existing drugs. The case I discussed was a fortunate man indeed in that such a pathway was identified, but most tumors are driven by many derangements in growth control, metabolism, migration, and the other hallmarks of malignancy described by Robert Weinberg. Worse, in many cases we don?t even have drugs that can attack many of the abnormalities that drive cancer progression. Then there?s the issue of tumor heterogeneity, which comes about because cancer is as good example of a disease as I can think of in which evolution due to natural selection results in incredible differences in the cancer cells in one part of the tumor compared to other parts of the tumor or in the tumor metastases. A ?targeted? therapy that targets the genetic abnormalities in one part of the cancer might well fail to target the genetic abnormalities driving another part of the tumor.

These, and many other reasons, are why we haven?t ?cured cancer? yet.

Of course, the main promise of the new targeted therapies, besides more precisely targeting the actual genetic abnormalities that drive the growth of specific tumors, is decreased toxicity. Whereas cytotoxic chemotherapy is like a blunderbuss that pummels and poisons cells, both normal and cancerous (it just poisons cancer cells more, which is why it works), ?targeted? agents are frequently portrayed as ?smart bombs? that attack only the cancer cells and produce little, if any, collateral damage. But is that true? There?s a whole lot of hype, but thus far the reality has been disappointing. If a new study just published online in the Journal of Clinical Oncology by a group at the Princess Margaret Hospital and University in Toronto, the answer is ?probably not.? The study?s title pretty much sums up the findings: The Price We Pay for Progress: A Meta-Analysis of Harms of Newly Approved Anticancer Drugs. However, as you will see, the way the results of the study (Niraula et al) are being reported are not exactly in line with what the study found. In other words, this study finds something that seems unexpected, part of which is a bit unexpected, most of which is not.

A bit of background is in order, and the introduction of the paper is a good place to start:

Although virtually all new drugs are tested in early-phase clinical trials designed to evaluate toxicity and tolerance, these trials are small and can detect only common toxicities. Most large RCTs are primarily not designed to detect differences in quality of life or toxicity of treatment between the study arms and are usually inadequately powered to do so. Furthermore, some anticancer drugs are approved on the basis of smaller, nonrandomized, unblinded clinical trials5 in which detection of toxicity can be severely impaired. Rare but potentially serious toxicities may not be detected in RCTs owing to a selection of patients with high performance status and minimal comorbidity or because of relatively short follow-up. The quality of reporting toxicity in RCTs has been criticized following evidence that some toxicities may be detected but inadequately reported.3,6 Increased treatment-related mortality associatedwith bevacizumab,7 increased cardiovascular morbidity of aromatase inhibitors,8,9 and increased risk of cardiopulmonary arrest with cetuximab10 were all either undetected or unreported at the time of drug approval but were described postmarketing. Therefore, efforts to monitor for toxicities after completing pivotal phase III RCTs are essential.11 The US Food and Drug Administration has encouraged a systematic approach to the collection and reporting of toxicity.12 Until such measures are implemented, there will continue to be concerns about failure of detection and reporting of serious and fatal toxicities.

In essence, the approval process for cancer drugs suffer from the same limitations as the approval process for all drugs does. The phase III studies used to support approval, although large, are not large enough to identify uncommon complications and toxicities or to do a fine analysis of differences in quality of life. Unfortunately, these sorts of issues are often not identified until approved drugs go into wide usage. Post marketing surveillance studies are useful for this purpose, but it?s a bit hit or miss whether such studies are done. Because anticancer agents, even targeted therapies, are directed at critical signaling pathways that control cell replication, death, mobility, and other key processes common in all cells, even targeted therapies are likely to produce at least some toxicity. Basically, all drug approval is a delicate balancing act between doing enough studies with enough patients to ensure that the drug tested is safe and effective versus cost and the time it takes to do the necessary studies.

What the authors hypothesized is that the experimental arms of studies used to support the approval of new agents would be associated with a higher rate of significant toxicity. The specific toxicities studied included treatment-related deaths, any toxicity severe enough that treatment had to be halted, and grade 3 and 4 adverse events (AEs). Grade 3 AEs include ?symptoms causing inability to perform usual social and functional activities,? while grade 4 AEs are potentially life-threatening and result in ?symptoms causing inability to perform basic self-care functions or medical or operative intervention indicated to prevent permanent impairment, persistent disability, or death.? To test their hypothesis, the authors identified 38 RCTs used to support approval of a drug for a cancer indication between January 2000 and December 2010, specifically the phase III randomized clinical trials used to support FDA approval of these drugs. A meta-analysis was performed, and correlations were made between the odds ratios for the three primary endpoints (treatment-related deaths, AEs resulting in treatment discontinuation, and grade 3/4 AEs) associated with the new drug and hazard ratios for overall survival (OS) and progression-free survival (PFS) associated with these drugs.

What the authors found can be boiled down fairly easily. Compared with control groups, the odds ratio (OR) for treatment-associated toxic deaths was elevated (OR, 1.40; 95% CI, 1.15 to 1.70; P peripheral neuropathy is a common complication of taxanes, so much so that many women I?ve operated on who underwent such therapy complain about numbness in their fingers, and one of the main drugs included in the meta-analysis was docetaxel (trade name Taxotere); so I?m not terribly surprised that this toxicity would come to the fore.

There are a number of other issues that were brought up with respect to this trial as well. The results I?ve just described were the overall results. The authors had several pre-planned subgroup analyses, and the results were a mixture of the aforementioned surprising and unsurprising results. The first surprising result is that the authors compared ? targeted drugs directed against a specific molecular target (specifically, the target population to be treated was selected by presence of a biomarker) versus drugs that are less specific (unselected target population, not based on presence of a biomarker). In this classification, docetaxel would be less specific because it is a general microtubule inhibitor that interferes with cell division. Its use is thus not chosen on the basis of an existing biomarker. In contrast, drugs like Tamoxifen or aromatase inhibitors would be considered ?targeted? because they target estrogen signaling and are therefore only used in women whose cancers make the estrogen receptor. Similarly Herceptin would be considered a targeted agent because it targets HER2 and is only used in women whose cancers make this particular oncogene protein product.

The findings were a bit unexpected in that, targeted therapies had a higher OR for the three end points than the ?less targeted? agents. However, I have a bit of a problem with this. Here?s a table describing the OR of the various endpoints for these two groups, with the 95% confidence intervals (CIs) in parentheses below each OR:

Group Toxic death

Treatment discontinuation

Grade 3/4 AEs

Specific targeted agents

Less specific agents

1.40

(1.22 to 1.60)

Notice that, for the specific targeted agents, the ORs are not statistically significantly different than 1.0 for toxic deaths and treatment discontinuation, but for the less specific agents, the ORs are statistically significantly greater than 1.0 for two out of the three endpoints. So, while it?s true that the risk of grade 3/4 AEs is higher for the targeted agents, the ORs for everything else are not. Personally, I?d call this a wash, or even potentially an indication that the less targeted therapies produce more AEs, as one would expect.

The next thing that the authors did that seemed to get lost in the news reports discussing this study. For instance:

The study showed the newer cancer drugs caused significantly more side effects, and more treatment-related deaths, than their older counterparts. ?You?ve got to consider both efficacy and toxicity in the picture,? said Susan Ellenberg, who has studied drug side effects at the University of Pennsylvania?s Perelman School of Medicine in Philadelphia.

She said the analysis wasn?t so surprising, and isn?t ?a big alarm.?

While it is correct that this isn?t a ?big alarm,? reporting this study in a blanket fashion as showing these newer drugs as causing significantly more side effects is simplifying the results of this study way too much. The reason, I would posit, comes from another pre-planned subgroup analysis that the authors did:

Agents were classified into four subgroups: targeted agent alone versus placebo or best supportive care (subgroup A); targeted agent alone versus nontargeted systemic anticancer therapy (subgroup B); targeted agent combined with nontargeted systemic anticancer therapy versus nontargeted systemic anticancer therapy alone (subgroup C); and chemotherapeutic agents (subgroup D). We compared the rates of treatment-related death, treatment discontinuation, and grade 3/4 AEs in each of these subgroups. In a separate analysis, we pooled the odds of hematologic and nonhematologic toxicities in each of the subgroups.

Overall, the finding of this subgroup analysis for the three endpoints of the study was described in the discussion thusly by the authors:

Our analysis shows that most newly approved anticancer drugs are associated with increased odds of toxic death, treatment discontinuation, and severe AEs compared with the standard treatment received by controls. There was an overall increase in the odds of toxic death, treatment discontinuation, and grade 3/4 AEs with the use of new agents versus the control groups except when targeted agents alone were compared with chemotherapy in the control arm. When individual toxicities were evaluated independently, the odds were increased to a greater extent for nonhematologic than for hematologic toxicities with the use of experimental agents compared with the controls.

In other words, when these new agents go head-to-head with existing cancer therapies, the OR for serious AEs is not elevated in the experimental group, while in groups for which the new agent was either tested versus placebo or best supportive care or combined with nontargeted anticancer therapies or with cancer chemotherapy the addition of the new drugs increased the chance of significant toxicity. This is about as expected a result as I can think of. Of course a new agent is likely to produce more toxicity than placebo; that is, if it?s an active drug. Of course adding a new agent to existing agents, be they traditional chemotherapy or other nontargeted agents will increase toxicity! It?s a general principle that an active drug will be more toxic than placebo and that adding another drug to an existing mix of drugs is likely to produce more toxicity! In fact, I?d be surprised if the investigators didn?t find that.

That?s the message that?s being lost in how this particular study is being reported. Now don?t get me wrong here. This study, admittedly, is disappointing. However, it?s disappointing not because it shows that the newer drugs are so much ?more toxic,? but rather because it suggests that, contrary to the hope and the hype, the newer drugs are probably not less toxic than existing drugs. Actually, the study doesn?t exactly show that. The numbers of studies looking at head-to-head comparisons of new drugs with established treatment regimens was too small. It does, however, strongly suggest that newer agents at the very least are not that much less toxic than existing drugs and might be more. It also confirms the long-known dictum in pharmacology that the more drugs you give at the same time the higher the chance of interactions and toxicity. Newer, ?targeted? drugs are not immune to that basic principle of pharmacology.

Another finding that seems surprising on the surface but really isn?t is that the observed toxicity of newly approved drugs did not correlate with how much they were able to increase OS or PFS. There is a misconception common even among doctors that the price we pay for more effective anticancer drugs is more toxicity, but that?s not necessarily true, and this study is simply more evidence to that effect. Another implication of this observation is simply to reinforce the point that we have to weigh toxicities very carefully versus incremental improvements in OS and PFS that result from adding new drugs to our cancer treatment regimens. There often comes a point where patients judge that the additional toxicity is not worth it for the amount of potential benefit that such drugs provide. In any case, it should not be surprising that toxicity and efficacy are largely independent properties of such drugs. As the authors themselves point out, efficacy is determined by the sensistivity of tumor cells and their microenvironment to the drug, while toxicity is more dependent on drug metabolism and clearance, as well as comorbidities, organ function, and concurrent medications, among other things.

What needs to be remembered about the new generation of more targeted therapies is that, unless they are used in patients whose tumors have the intended molecular target and are thus likely to be sensitive to the medication, the chances of benefit decline while the chances of toxicity increase. That is why, for instance, we do not give Herceptin to patients whose tumors do not make HER2 and do not use Tamoxifen or aromatase inhibitors in patients whose breast cancer is ER(-). For us to realize the maximum benefit at the lowest cost in terms of toxicity from these new medications, in addition to biomarkers that predict response (such as the aforementioned ER and HER2 markers) but biomarkers that also predict toxicity.

Finally, it?s important to note that, as incredible and copious as it is, all this genomic data doesn?t help us that much?yet. The reason is that we don?t yet know what to do with this ?firehose? of a data stream, how to apply the data contained therein to guiding patient care in order to cure more patients and prolong the survival more of those who cannot be cured. I actually believe that one day we will have the tools and knowledge to do that, such that I envision a day when we will use tumor biopsies and blood to construct comprehensive genomic, transcriptomic, and proteomic profiles of individual patient tumors and use them to target the molecular derangements that drive them. However, I am under no illusion that reaching that day will be easy, cheap, or without other cost. Studies like Niraula et al keep us humble.

Autopsy: late Falcons star had brain disease

(AP) ? Former Atlanta Falcons Safety Ray Easterling had a degenerative brain disease associated with repeated concussions, an autopsy report concludes.

The medical examiner's report found that Easterling's brain had signs "consistent with the findings of chronic traumatic encephalopathy," a progressive degenerative disease that can be caused by multiple concussions. Easterling died of a self-inflicted gunshot wound in April. He was 62.

The autopsy report was provided Friday to The Associated Press by Mary Ann Easterling, who is among thousands of plaintiffs who have sued the NFL, claiming the league failed to properly treat players for concussions and attempted to cloak any links between the violent sport and brain injuries. The autopsy's findings were first reported by The New York Times.

"I was expecting it," Easterling said. "It verifies all of our suspicions, Ray's included."

Easterling helped lead the Falcons' stout defense in the 1970s. He played from 1972 to 1979 as a member of the "Gritz Blitz" defense in 1977 that set the NFL record for fewest points allowed in a season.

After his career, however, Easterling suffered from dementia, depression and insomnia, Mary Ann Easterling said.

"I feel affirmed and I feel at peace," she said of the autopsy report. "It made my heart hurt even more to know he suffered through that."

A Richmond neuropsychiatrist who had treated Easterling for several years said the autopsy report confirmed the specific neuropath change that is a signature sign of chronic traumatic encephalopathy.

"It distinguishes it from Alzheimer's disease or any other type of degenerative dementia," Dr. Gregory O'Shanick said. Easterling's brain injury, he said, were reflected in a "series of cognitive and behavioral changes."

Easterling's wife has said her husband had lost her ability to focus, organize his thoughts and relate to people.

Hackers could haunt global air traffic control: researcher

Air traffic control software used around the world could be exploited by hackers to unleash squadrons of ghost planes to befuddle those entrusted to keep the skies safe, a security researcher said Thursday.

Cyprus-based Andrei Costin demonstrated his findings at a Black Hat gathering of cyber defenders that ends Thursday in Las Vegas.

"This is for information only," Costin said as he outlined how someone with modest tech skills and about $2,000 worth of electronics could vex air traffic controllers or even stalk celebrities traveling in private jets.

"Everything you do is at your own risk."

Costin's target was an ADS-B system in place for aircraft to communicate with one another and with air traffic control systems at airports.

The system, which has been rolled out internationally in recent years in a multi-billion dollar upgrade, was designed to better track aircraft so airport traffic can flow more efficiently.

A perilous flaw is that the system is not designed to verify who is actually sending a message, meaning that those with malicious intent can impersonate aircraft either as pranks or to cause mayhem, according to Costin.

"There is no provision to make sure a message is genuine," he said.

"It is basically an inviting opportunity for any attacker with medium technical knowledge."

Air traffic controllers faced with a signal from a fake airplane resort to cross-checking flight plans, putting relevant portions of air space off limits while they work.

"Imagine you inject a million planes; you don't have that many people to cross-check," Costin said. "You can do a human resource version of a denial of service attack on an airport."

Denial of service attacks commonly used by hackers involve overwhelming websites with so many simultaneous online requests that they crash or slow to the point of being useless.

Aviation agencies are adept at identifying and locating "rogue transmitters" on the ground, but not at countering signals from drones or other robotic aircraft becoming more common and available, according to the researcher.

Another danger in the new-generation air traffic control system, according to Costin, is that position, velocity and other information broadcast by aircraft isn't encrypted and can be snatched from the air.

"Basically, you can buy or build yourself a device to capture this information from airplanes," Costin said.

He listed potential abuses including paparazzi being able to track private jets carrying celebrities or other famous people.

Costin showed how a friend was able to identify a plane broadcasting the identification numbers of Air Force One, the military jet used by the US president, and plot it on a map on an iPad.

"It can be a very profitable business model for criminals to invest a small amount of money in radios, place them around the world" and then sell jet tracking services or information about flights, the independent researcher said.

"If it was Air Force One, why does Air Force One show itself?" Costin wondered aloud. "It is a very high profile target and you don't want everyone to know it is flying over your house."

There are websites with databases matching aircraft registration numbers with listed owners.

House GOP rejects Obama plan for offshore drilling

WASHINGTON (AP) ? In an election-year swipe at President Barack Obama's energy policies, the Republican-led House has voted to revoke the president's five-year plan for offshore drilling, replacing it with its own plan calling for more ambitious oil and gas development off the U.S. coast.

The legislation will likely go nowhere in the Senate and the White House has issued a veto threat, but as with the tax and regulatory bills the House is also taking up this month, it puts lawmakers on the record on the issues that divide the two parties.

Republican Doc Hastings of Washington state, who chairs the Natural Resources Committee, said the bill offers lawmakers a choice between Obama's restrictive plan and the far more expansive Republican version which opens up drilling along the Atlantic and California coasts.

Designing or revamping a new home is something that can be both nerve wrecking along with being highly exciting. Often named as a dream paradise, almost all your savings are squandered on designing the same. Be it the exterior of your house or the interior including the paint, textures, furniture or designing the ambiance, every aspect and detailing is given due attention. With considering these points, can you afford to avoid the most essential aspect of your abode?

A question that might be striking in your mind right now is that every aspect has been covered, so what is missing out. An indispensable part of any housing project is the electric panel. For a layman who has no idea about the term at all, it is a huge metal box that consists of fuses and circuit breakers that makes the flow of electricity to the houses or the companies feasible. Quite often it has been compared to a family tree that has branches which are usually the wirings. The working procedure of the panel include the flow of current into the wirings and then into the divisions providing electricity flow in every part of the house

There are other names with which this panel is known and these are :-

  • Circuit breaker panel :- The main function of this panel is to shield your home if there is a doubt or a chance of electric overload. Just in case a situation arises where it is realized that there is overload of the power because of a fault in the wiring or any other reason, the breaker panel will automatically trip that will result in the electric shut down. There can be chances where the electricity of the entire house will be shut off or just few appliances, it would highly depend on the fact that which part of the house is sucking in more energy.
  • Breaker boxes :- A universal procedure to provide your home with additional energy is through breaker boxes. It is however known to be the most complex procedure used till date. Adjoining each and every circuit to breaker box is highly essential since there might be chances where a breaker might have tripped and a particular part or phase of your house is not able to function.

There are also certain aspects that should be kept in mind which indicate when does your amp panel need a revamping. It is significant to keep an idea on these points since these will give you an idea that it is high time you should think about the security of your home and yourselves as well.

Let's give a glance to these points:

  • You should consider changing an electric panel when you are bringing in a bigger entity in your home whether it is a bigger refrigerator or an air conditioner.
  • If you are planning to a new place that has an old panel, it is mandatory for you to swop the older one.
  • Totaling up new facets in your house that includes installation on hot water tub or a power appliance in the same.
  • If you have renovated your home and added up another room or modified your kitchen making it a modern one.

A Far-Out Solar System That Looks Like Ours

From far enough away,, our solar system looks 2-D. All eight planets still orbit on nearly the same horizontal plane, which matches up with the plane of the sun's equator. It?s what makes our home system look like a flat, spinning disc, and also a sign that our system hasn?t experienced anything too chaotic since its formation more than 4 billion years ago.

Until now, many astronomers thought that comparatively few star systems end up looking like ours. The planet-hunting Kepler telescope has spotted many systems in which the arrangement of worlds is askew rather than orderly. But now, thanks to new research being published in the journal Nature this week, scientists say there might be many more star systems that look like ours.

The three planets of a star now called Kepler-30 all appear to be in orbit around their star on the same horizontal plane, according to Roberto Sanchis-Ojeda, the lead author on the study and a Ph.D. candidate at MIT. "The physical conditions of the new system are such that it looks pretty much like our system."

Bumpy Beginnings

The birth of a solar system can be a messy process. The forming star?s gravitational pull attracts dust, gas, and rock that form a ring around the newborn. After billions of years, the chaotic ring eventually starts to differentiate itself into the component parts of what we recognize as a solar system.

Round planets and oddly shaped asteroids carve out their own orbits in the newly formed planetary neighborhood. But usually, Sanchis-Ojeda says, the results aren?t perfectly clean. Maybe a few planets scatter to one side or fall into a different orbit. Sometimes, if two planets form close together, the larger world?s gravitational forces can push the weaker out of orbit or off track. A rogue asteroid or comet could quite literally knock a planet out of whack.

Turbulence rocked our own solar system in its early days. For instance, the most likely explanation for the moon?s origin is that a Mars-size world struck the forming Earth, expelling the material that coalesced into our satellite. Nevertheless, the forces that wreaked havoc on solar systems around the galaxy early in their lives seem to have left ours relatively unscathed?despite the dangers, we wound up with eight planets on a horizontal plane.

Looks Like Home

Sanchis-Ojeda and his team were on a mission to find solar systems shaped like ours. The researchers used Kepler data to pick out certain star systems that fit within their parameters. Specifically, they wanted to study close young active stars with plenty of starspots (the equivalent of sunspots but on another star). These cooler areas on the surfaces of a star can act as markers for where a star is in its rotation; if you see the same starspot twice, you know the star has completed a revolution. Even though these spots fade with time, they stick around long enough to get a good read on the spin of a young star.

Sanchis-Ojeda saw three planets in the Kepler-30 system cross in front of, or transit, the same starspot?one right after the other. This meant that either the planets just happened to line up at that lucky moment before continuing their orbits on different geometric planes, or they were formed like our solar system?from a horizontal ring of gas and space dust. Sanchis-Ojeda hoped for the latter.

His confirmation came when the planet closest to the star transited the same starspot twice in the time Kepler had its eye on the system. The second transit all but affirms that the three planets didn?t just align by chance during their dissimilar orbits, he says?they must be on the same plane.

Kepler-30 is particularly important because its planets spin at an angle we can see from Earth. Our perspective and the placement of the relatively close solar system match up, making it easier to observe the transits. Kepler-30 is also quite close to the sun in mass and radius.

More Out There

Drake Deming, a professor of astronomy at the University of Maryland and author of a commentary accompanying the new study, says the researcher?s methods for tracking planetary orbits could eventually become a new standard for planet hunters, especially those looking for more solar systems like ours in the Universe.

Before Kepler, most of the exoplanets discovered were hot Jupiters?huge gaseous planets that were usually orbiting their stars in wildly oblique patterns. Astrophysicists know a lot about these "hot Jupiter" systems because they were relatively easy to study without space-based telescopes like Kepler.

But now researchers are finding more evidence that multiplanet systems such as ours are far more common than the solitary gas giants. "New studies have shown that indeed only one out of every 100 stars have a hot Jupiter," Sanchis-Ojeda says. "There are many more small planets close to a host star."

Although the Kepler-30 planets probably don?t hold life?all three are too close to their home star?they represent another step closer to finding a solar system that might, Deming says. The next step could be a new NASA program that, if funded in the fall, would take Sanchis-Ojeda?s methods even further. The Transiting Exoplanet Survey Satellite (TESS) is a Kepler-like satellite, but instead of focusing on a small sliver of the sky, TESS would survey the entire sky for nearby solar systems that could be similar to ours. "TESS would be an all-sky version of Kepler," Deming says. "We?ll have a better chance of finding nearby stars. It might be the future of exoplanetary science."

Is Social Media Elitist?

You already know social media can help get you a job, but new research suggests that's much more likely to be the case for higher-level managerial jobs than it is for low-paying jobs.

Researchers at North Carolina State University show that informal social networks such as Facebook and LinkedIn play an important role when it comes to finding jobs in both the United States and Germany, but those networks are significantly more important for high-paying jobs in the United States, which may contribute to economic inequality, the researchers speculate.

The researchers looked at nationally representative survey data from the United States and Germany to compare the extent to which people find new jobs through "informal recruitment." Informal recruitment via social media occurs when a person who is not looking for a new job is approached with a job opportunity through social connections.

The study shows that, on average, informal recruitment is significantly more common in Germany, where approximately 40 percent of jobs are filled through informal recruitment, ?as opposed to approximately 27 percent of jobs in the United States.

However, the jobs people find through informal recruitment in the United States are much more likely to be high-wage managerial positions. Specifically, in the United States, the odds that a job will be filled via informal recruitment increase by 2 percent for every dollar of hourly wage that the job pays.

For example, the odds that jobs paying $40 per hour ($80,000 per year) will be filled through informal recruitment are about 66 percent better than the odds that a minimum-wage job ($7.25 per hour) will be filled through informal social media recruitment.

By comparison, the researchers found that wages in Germany did not appear to be linked to how workers found their jobs.

?The finding was surprising since in the United States a free market with minimal labor restrictions is believed to value merit over social connections, said Richard Benton, a Ph.D. candidate at NC State who co-authored the research.

?"Ultimately, this suggests that U.S. economic institutions offer greater rewards to sponsorship and nepotism than what we see elsewhere, which could help to explain why inequality is so extreme here," said Steve McDonald, an associate professor of sociology at NC State and lead author of the paper.

The paper was published online in the journal Social Forces. The paper was co-authored by David Warner of the University of Nebraska-Lincoln. The research was supported by NC State?s College of Humanities and Social Sciences.

Colonoscopy screening markedly reduces colorectal cancer incidence and death

ScienceDaily (July 24, 2012) ? A study from researchers in Switzerland found that colonoscopy with polypectomy significantly reduces colorectal cancer incidence and colorectal cancer-related death in the general population. A total of 12 colorectal cancer cases were identified in the screening group of 1,912 patients and 213 cases of colorectal cancer were found in the non-screened group of 20,774 patients. One of the 12 persons of the screened individuals with a colorectal cancer and 51 of the 213 persons of the non-screened individuals with a colorectal cancer died because of their cancers. The study appears in the July issue of GIE: Gastrointestinal Endoscopy, the monthly peer-reviewed scientific journal of the American Society for Gastrointestinal Endoscopy (ASGE).

Colorectal cancer (CRC) has a very high incidence in Switzerland as well as in other European countries and is the second most frequent cause of cancer-related deaths in Europe. It is detected in approximately 413,000 people in Europe every year, half of whom die because of the disease. Therefore a need exists for efficient strategies for prevention and early detection of CRC. Colonoscopy with the possibility of an immediate polypectomy is a recommended and preferred screening method because polyps (growths in the colon) can turn into cancer over the course of years to decades. Removing polyps during a colonoscopy prevents that polyp from becoming cancerous.

"In contrast to earlier CRC screening studies that used colonoscopy, this population-based closed cohort observational study aimed to obtain complete and comparable data on CRC incidence and CRC-related mortality after a single screening colonoscopy compared with no screening, while taking into account the potential differences in risk profiles between the screened and non-screened participants," said study lead author Urs A. Marbet, Cantonal Hospital of Uri, Altdorf, Switzerland. "We found that colorectal cancer screening by colonoscopy markedly reduces not only the incidence of colorectal cancer, but also cancer-related death. We are unaware of any other long-term prospective study assessing the role of colonoscopy screening for the reduction of colorectal cancer incidence and mortality in a well-defined, population-based setting under real-life conditions."

Methods

The researchers' objective was to compare the incidence of and mortality from CRC among individuals screened by colonoscopy and non-screened individuals. The study involved 1,912 screened patients and 20,774 non-screened control participants. It was a closed cohort study in a population-based setting in a precisely defined area with a low level of population migration (a mainly rural area of Switzerland) from June 1, 2000 to June 1, 2001. Colonoscopies were performed by 11 board-certified gastroenterologists, including three local gastroenterologists who were supported by 10 gastroenterology trainees from the University Hospitals of Basel and Zurich; each of the trainees had performed at least 200 procedures. Study participants were aged 50 to 80 years old.

CRC cases in this closed cohort study were prospectively collected during the screening period of one year and the follow-up period of six years (June 1, 2001 to May 31, 2007). The main outcome measurements included follow-up data that were corrected for negligible migration balance in the area, and included tumor characteristics and risk or protective factors, age and sex, participation in general health screening examinations, history of CRC in a first-degree relative, smoking status, body mass index, frequency of sports activity, eating habits, and patients' professions. Colorectal cancer and cancer-related death was recorded for all participants. Statistical comparisons were made between the screened and non-screened patient groups.

Results

Polyps were found in 565 of the 1,912 screened individuals (29.6 percent), including 374 persons (19.6 percent) with adenomas (precancerous polyps) by histology. All polyps found during colonoscopy were removed during the procedure, except for very small lesions in the rectum, or later by surgery (surgical polyp removal occurred in seven cases -- 0.36 percent). Overall, 1,279 polyps were removed. Colorectal cancer incidence was significantly reduced by colonoscopy screening. Overall, 225 colorectal cancers were detected.

A total of 12 CRC cases were found in the screened group (0.6 percent of the screened persons), including one which was found during follow-up 60 months after the initial screening (0.05 percent of screened persons). In the non-screened group, there were 213 cases of CRC (1.0 percent). None of the non-screened patients, of whom five presented with synchronous cancers, and none of the persons who had been excluded from screening, had previously undergone a colonoscopy.

A total of 72 percent of the screened-group cancers (66.7 percent including the one detected during follow-up) and 19.7 percent of the cancers in the control group were at Tumor (T) stage one or two. One of the 12 persons of the screened individuals with a colorectal cancer and 51 of the 213 persons of the non-screened individuals with a colorectal cancer died because of their cancers. Colorectal cancer-associated mortality was clearly lower in the screened group. The risk profile in the screened group was comparable to that in the general population. Risk factors such as lifestyle, smoking, and body mass index, as well as family history, were similar in both groups. "Blue-collar workers" had a higher incidence of CRC compared with "white-collar workers." The risk factors identified for CRC were a positive family history and smoking. The researchers noted that possible limitations of the study included the relatively low number of participants, confounding factors related to the ethnicity of the subjects, and that it was a non-randomized study.

The researchers found in this closed cohort study a substantial reduction in the incidence of colorectal cancer and colorectal cancer-related mortality in a sample of asymptomatic individuals undergoing a single colonoscopy screening compared with non-screened individuals.
